Normalization of Deviance

The term normalization of deviance was coined shortly after the space shuttle challenger disaster. It was discovered that ongoing, dangerous practices were allowed to occur during the project and even right up to takeoff. Allowing these dangerous practices to occur, in time became normal practices. If it can happen at NASA, it could happen in your organization.
